Coronectomy
A coronectomy is a dental procedure that removes only the top crown of a wisdom tooth, leaving the roots in place to avoid nerve damage. Primarily used for lower (mandibular) third molars sitting too close to the inferior alveolar nerve, this technique reduces risks like permanent numbness, infection, and fracture.
When is a Coronectomy Recommended?
Used when panoramic X-rays or CT scans show the root is dangerously close to the nerve.
Primarily performed on impacted lower third molars (mandibular).
Specifically, it is chosen when removing the entire tooth could cause permanent numbness of the lip, chin, or tongue.
Under local anesthesia or IV sedation, the dentist cuts the gum, removes the crown, and leaves the root. While reducing nerve injury, risks include a 3–14% chance of needing future surgery to remove the remaining root if it migrates or causes issues.
What to Expect After the Procedure
Recovery
Similar to or easier than a traditional extraction, with pain typically reducing after 2–3 days.
Long-Term Results
In most cases, the roots remain buried in the bone, and in about 25% of cases, they "migrate" away from the nerve.
Contraindications
Not recommended if the tooth is highly infected, has a very thin root, or if the tooth is loose
